You must complete the SFDPH online compliance training and e-sign the confidentiality and code of conduct agreements before logging on to a DPH system. Removing a user: If a previous user has left your organization, you may use the same form as above to delete their account. For deletions, we only need the first page completed and submitted.

Commonly Asked Questions

I am having trouble with my password. What can I do?

If you have forgotten your password or if your account has been locked out because you tried logging on too many times, you can call the SFDPH Help Desk at (628) 206-7378 and ask for a password reset.

Your password will automatically expire and must be changed every 90 days. You will not be able to log on to Epic Carelink with an expired password. To change your password, follow these steps:

Enroll in Password Reset:

  1. Using a web browser (Google Chrome, Mozilla Firefox, Microsoft Edge or Apple Safari, for example) go to https://sfdph.org/dphuser

  2. Click on Enroll in Password Reset.

  3. Log on using the username and password listed below.

  4. Add at least one phone number that can receive SMS Text messages.

  5. Click on Submit.

Change your password:

  1. Using a web browser (Google Chrome, Mozilla Firefox, Microsoft Edge or Apple Safari, for example) go to https://sfdph.org/dphuser

  2. Click on Reset Password

  3. Log on using the username and password listed below.

  4. Click on Send Verification Code.

  5. Enter the code received on your registered device.

  6. Enter your new password in the fields shown.

    • Your password must meet the following requirements:

    • At least 10 characters long

    • Contain an Upper case letter, a lower case letter, a number and a symbol character.

  7. Click on Reset Password.

Is there any required online training for SFDPH Epic CareLink?

What if I have users at my site who also works at a DPH (ZSFG, LHH, etc.)?

Users who also work at a DPH site will only be given access to Epic and not SFDPH Epic CareLink. Complete the form as you would for other users, but indicate that they have an existing DPH account by checking the appropriate box on the form.

What is the difference between the Roles listed on the form?

Support: Can review charts, place orders, follow status of authorized orders. If no selection is made, this role will be selected.

Coordinator: Can create charts, review charts, place orders, follow status of authorized orders. Must follow a strict procedure to prevent the creation of duplicate charts.

Site Admin: Responsible for reviewing the list of users every 3 months and disabling accounts for separated staff.

Physician: Credentialed provider that can authorize orders. Must supply NPI and DEA License numbers on the form along with their specialty and title.

Applying for Avatar credentials

Avatar User Credentials are issued by applying directly to DPH. For the forms, please visit the Avatar Documentation Page on SFDPH website (https://www.sfdph.org/dph/comupg/oservices/mentalHlth/BHIS/avatarUserDocs.asp ). Along with the on-boarding forms, this site has access to instructional videos and additional resources.

Everyone is required to attend training before getting their login information. Watching the instructional videos is insufficient to meet this requirement.

Everyone is required to submit a certification form that includes Taxonomy Code and NPI number. Staff will need to go the NPI Registry (https://nppes.cms.hhs.gov/ ) to register and have an NPI number assigned to them. The certification form is not considered complete without the NPI/Taxonomy.

Each form has a fax number on it (Compliance 415.252.3032 / Avatar Accounts 415.252.3008). Compliance receives the certification form and electronic signature agreement.  Avatar Accounts receives the account request form.  It takes about 10 business days to complete the process (including creation of the Webconnect account if needed).  Account information is usually emailed to the requesting supervisor.
