We are the SFCCC

The San Francisco Community Clinic Consortium develops innovative programs and advocates for policies that increase access to quality community-based primary health care. We work to ensure that people of all income levels have access to health care that is:

  • Comprehensive, coordinated, and efficient
  • Culturally and linguistically appropriate
  • Focused on patient needs

The SFCCC provides primary care services to more than 10% of San Francisco’s population. We work with providers citywide to coordinate primary care with specialists, hospitals, and other services. We serve overlooked populations such as homeless San Franciscans, immigrant communities and seniors.

Affordable Care Act Expands Health Coverage

After years of advocacy, the Affordable Care Act (ACA) created new health insurance options and protections. The ACA offers many of our uninsured patients the opportunity to enroll in Medi-Cal or Covered California, the state’s health benefits exchange. Working with city departments, as well as local, state, and national elected officials, the SFCCC provides the link between public policy and day-to-day clinic operations.

The SFCCC and our partner clinics are leaders in ensuring that patients receive coverage. We are one of the first groups of “master trainers” certified by the state to help people enroll in new programs. The SFCCC clinics have provided care to the majority of those enrolled in Healthy San Francisco since it was established. After ACA enrollment started, we immediately began to transition all who were eligible to full health insurance programs. We will continue to advocate for the continuation and expansion of Healthy San Francisco for those who are ineligible for full ACA coverage.

Patient-Centered Health Care Tailored to Each Individual

We worked with Kaiser Permanente to implement Kaiser’s PHASE (Preventing Heart Attacks and Strokes Everyday) program in SF clinics. Today, our partners are designing programs that fit into patients’ complex lives, improving medication adherence while supporting healthy lifestyle changes.

Sam, age 62, lives in a small apartment with several cousins and works six days a week as a restaurant dishwasher. He was diagnosed with Type II Diabetes a few years ago, with an initial hemoglobin A1c (HbA1c) level of 11. HbA1c indicates how well a person with diabetes is managing their disease. Diabetics typically strive for a result of 6 or lower and Sam’s result indicated poor control.

Sam eats most meals at the restaurant where he works in order to save money, so his provider from a SFCCC member clinic talked with him about the best choices from the restaurant menu and a plan for at-home meals focused on salads, since he didn’t have consistent access to a stove. The exercise plan they developed together included options Sam could afford, such as basketball with friends and walking more frequently. Sam’s provider also developed a unique insulin regimen tailored to his long hours at work.

After implementing this patient-centered plan, Sam’s HbA1c result improved, dropping to 8.4. The respectful and practical approach of clinic staff increased the chance that he will continue improving his health and quality of life, and reduce his HbA1c to below 6.

Healthy LifeStyle Changes

One Thursday morning, George came into our Street Outreach Services medical van for a blood pressure check while we were making one of our weekly stops in the Mission District. His blood pressure was 183/119, Stage II Hypertension – almost high enough to send him to the emergency room.

We set him up with a primary care physician at an SFCCC partner health center and discussed lifestyle changes he could make. He now has a doctor managing his high blood pressure with medication, he has cut back on alcohol, and he has reduced both his cigarette smoking and sodium intake. He told us that we gave him that last push he needed to make changes in his life. George came back to us a few months later and we took his blood pressure. His reading was 120/78 – right on target! George thanked us with a big smile, and told us we saved his life.

13 Languages

The SFCCC partners provide service in more than 13 languages and multiple dialects. This is just one example of our sensitivity to race, gender, culture and sexual identity in providing high-quality care.

600 Workers

The SFCCC organizations employ 600 workers, bringing in $90 million to support our clients citywide. Less than 15% of the SFCCC’s funding comes from the City of San Francisco.

SFCCC – A Glimpse Inside

It’s a typical Friday, and the SFCCC staff and volunteers are busy with advocacy and patient care. CEO John Gressman is in Washington, D.C., talking to House leadership about Federal funding for community health centers across the country.

Back in San Francisco, the SFCCC staff members meet with the directors of our 11 clinics and Community Benefit staff to discuss using hospital-provided grant funds to improve quality, patient flow and the patient experience at our clinics. Blocks away, Merrill Buice, the SFCCC’s leader on public programs outreach and enrollment, trains 50 health clinic staff members as Certified Enrollment Counselors to assist patients with enrollment in Medi-Cal and Covered California.

At the office, a long-time volunteer and member of the SFCCC’s patient advisory committee assembles care packages for the homeless outreach mobile clinic, while the quality improvement department gathers and analyzes data from each clinic to deliver shorter wait times, more cost-effective treatments, and better integrated care.

Every day, the SFCCC is creating innovative programs that improve the lives of 100,000 low-income San Franciscans and increase the viability of our community clinics. The SFCCC’s combination of leadership and community expertise enables all San Franciscans, regardless of income, to have high-quality, responsive and comprehensive health care.
